I understand that proof of identification is not a requirement for membership, however, I solemnly swear that the information I am providing above is correct and true to the best of my knowledge. I understand that if this information is false, my membership is automatically void and I will be entering into a private premise without consent.

OUR MISSION STATEMENT

The name of the Society is CLUB ALPHA(hereinafter “Club Alpha” or “the Club” )The purposes of Club Alpha are:

• a. The mission of Club Alpha is to revolutionize the art of relaxation and entertainment by returning to a level of class, privacy and sophistication not usually found in the any alternatives.
• b. To celebrate positive sensual/sexual interaction among open-minded consenting adults who participate in a sensual lifestyle (the “Lifestyle”):
• c. To do all other such things that are incidental and ancillary to the attainment of the purposes of Club Alpha.

MEMBERSHIP

Members – Individual Members must be persons who are:

• a. Over the Age of Majority as defined by the Age of Majority Act of Ontario. That age is currently at ninteen (19)
• b. Capable of understanding and freely consenting to the matters set fourth in this document, the Liability, and the Privacy Agreement.
• c. Individual Members are entitled to attend Club Alpha's functions and events so long as their membership is in good standing. To be an individual in a good standing, he or she must obey the member's code of conduct, pay membership dues and not violate any of the provisions set fourth in this document, the Liability Waiver, and the Privacy Agreement.

MEMBERSHIP RULES

• 1. No minors. No members shall invite or bring anyone under the age of Majority to Club Alpha's function or event.
• 2. No infections. No member shall attend a Club Alpha function or event if they know or suspect that they may be capable of spreading a sexually transmitted infection or disease.
• 3. Respect for premises and facilities: Members shall respect the premises and facilities that Club Alpha functions or events are taking place in, and will immediately leave if asked to do so by a Founding Member or a Club Alpha employee or representative.
• 4. Employees and representative: 1. Only Club Alpha employees and appointed representatives shall represent the Society's interest. Members shall not represent Club Alpha in any official capacity without specific empowerment from a Founding Member.
• 5. Respect privacy: All members shall respect the privacy of other Members and the Club Alpha, adhere to the Privacy Agreement in this document, and not violate the Privacy Act of Ontario by taking unauthorized images or pictures.
• 6. Respect all laws: 1. All Members shall obey all laws, including refraining from the use of any illegal drugs while at Club Alpha's facilities.
• 7. Respect for appointments: Members who does not show up for a booked appointment will lose the privilege to book appointments with any hostesses and will be received at the Club Alpha on a walk in basis only.

MEMBERSHIP REMOVAL

In rare instances, individuals may be asked to leave the Club. Members may be removed at the sole discretion of a Founding Member. Members may be removed for any reason, but are generally removed for repeated violation of the member's code of conduct. After one year, a member may be readmitted with the approval of a Founding Member.

I HEREBY APPLY FOR MEMBERSHIP I HAVE READ THE ABOVE AND AGREE WITH IT'S CONTENTS ENTIRELY, AND ARE SIGNING UNDER MY OWN FREE WILL AND CONSENT.

WHEREAS:

Representation of Good Faith: I represent those that I approach the BRASS SOCIAL CLUB (“The Club”) and it's member and hostesses (the “Members”) in a sincere desire to learn more about open-minded consenting adults who participate in a sensual lifestyle (the “Lifestyle”) and to possibly participate in the same. I intend no harm, interference or disruption to the Club, to the premises or to the lives of any who adhere to its precepts (rule of behaviour).

In consideration of being permitted to join the Club, attend an event, or to enjoy the facilities or services provided by Club Alpha, I agree as follows:

1. Protection of Privacy. I recognize that the philosophy of Club Alpha, while legal, is controversial, and that activities conducted in pursuance or it as as well, and I represent that I am not offended by nudity, sensuality or sexual interaction in my presence in a private setting and I will respect the privacy of the Members, including but not limited to the following:

• a. I will not divulge the identity of anyone as a Member of Club Alpha to anyone without their permission.
• b. I will not discuss or disclose any of my experiences in Club Alpha with anyone outside of Club Alpha without the permission of the Founding Member, except that I may discuss my experience in a way which does not permit the Club or the Members to be identified.
• c. I will not disclose the location of the Club without the consent of Club Alpha.
• d. I will not use cameras, tape recorders, cell phones, any recording devices, or take photographs in Club Alpha's facilities without the express written permission of Club Alpha. If I record a private conversation of a beneficiary without consent, I shall be subject to liability under the Privacy Act of Ontario and common law rights of privacy

2. Provision as to police officers and other agents/officers. Club Alpha understands that police and other agents/officers have a duty to enforce laws and to investigate suspected violation of them and that the constitution Act of Canada limits the right of officers to do certain activities without consent. I warrant that, if I am a police officer or government agent of any kind, I am present at Club Alpha only in my own private and individual capacities. I warrant that, if the foregoing statement should prove to be false, I am investigating any of the Members without disclosing my official capacity and purpose, and that my presence on Club Alpha premises, or those of it's affiliates or members, is an entry into private premises without consent.

3. Provision as to members of the press. 1. I represent that, if I are a member of the press (including television), I am present at Club Alpha, only in my own private, individual capacities. I warrant that, if the forgoing statement proves to be false, that I am here at the direction of my employer and am acting within the scope of our employment; that I am violating the right of the Members to privacy under common law.

4. Indemnity. I agree to indemnify and hold harmless the Members and Club Alpha for any harm resulting from our violation of this agreement, particularly if I knowingly give information to a journalist or law enforcement officer. The measure of damages for purposed of indemnification shall be the same as in an action sounding in tort for invasion of privacy, plus any lost wages resulting from loss of employment due to a violation of this agreement. The Members are entitled to reasonable attorney's fees, both for the enforcement of this agreement and for the prosecution or defense by the Members of any other legal action initiated as a consequence of our breaching this agreement. The pursuit of any other remedy, including tort, is permitted in addition to the above provisions.

5. Duration and limitation. This agreement shall continue in force and apply to all attended events and functions and communication between Members.

DISCLAIMER Club Alpha (or the “Club), the founding members and its members take no responsibility and will not be held responsible for any injuries, personal loss or accidents that may occur as a result, either directly or indirectly, of becoming a member of Club Alpha, or during Club events.

Understand that you are considered an ‘invited guest’ and a licensee and that participation in all Club events and functions is at your own risk.

By becoming a Member of the Club or participating in any Club events or functions, you are taking responsibility for your own safety and well-being.

Understand that if you are offended in any way by sensuality, sensual touching or sexual expressions, the environment offered at the Club is not for you and you should not become a member. Also, becoming a member of Club Alpha without the knowledge or consent of your partner or spouse may be harmful to your relationship.

Consumption of alcohol is at your own risk and you are responsible for any occurrences as of result of your intoxication.

MEMBERS RELEASE AND WAIVER OF LIABILITY INDEMNITY AGREEMENT

The member that signs below is referred to as “I” or “Me” or “Myself”. I hereby acknowledge that we are voluntarily reading this document and joining Club Alpha and participating in its activities, hereinafter referred to as “Activities”. I will be personally responsible for any and all damages and personal injury of any kind, including death. I am aware that the Activities may involve risk of injury, infection, disease, death, or property damage and I am voluntarily participating in these Activities with full knowledge of the risks.

As lawful consideration for being permitted by the Founding Members and Club Alpha to engage in the Activities, I do, my heirs, executors, administrators, legal representatives, guardians and assigns, hereby release, waive, discharge, and relinquish Club Alpha and the Founding members or any of its officers, agents, servants, volunteers, leaders, activity participants, or employees, herein referred to (collectively the “Hosts”) for any action or causes of action for personal injury, property damage, or wrongful death which may hereafter arise from the Activities, alcohol consumption, or any pursuit incidental thereto wherever or however said pursuit may occur and for any period said Activities and pursuits may continue.

I further agree that under no circumstances will I prosecute or present any claim against the Hosts for any causes of action, for personal injury, property damage, or wrongful death, whether the same shall arise by the negligence or non-intentional conduct of any said Hosts from the Activities or any pursuit of incidental thereto.

I hereby agree to indemnify, save and hold harmless the Hosts and each of them for any loss, liability, damage or cost (including legal fees) which I may incur as a result of injury, death, or property damage to the undersigned, or from suit from such personal injury, death, and/ or property damage to undersigned. If I am a guest to Club Alpha, I further agree to save hold harmless the Hosts and each of them from any loss, liability, damage or cost (including legal fees) which I or any of our invitees may incur as result of injury, death, or property damage to the undersigned, or from suit from such personal injury, death and/or property damage.

This agreement is intended to be as broad as permissible under the laws of the Province of Ontario, Canada, and this agreement shall be interpreted under the laws of the Province of Ontario, Canada. If any portion of this agreement is invalid and/or is declared to be invalid by a Court of Law, the balance of the Agreement shall continue in full force and effect.

Attending any Club Alpha event or function signifies that I (and any guests we bring) have read and voluntarily agree to the release and waiver of liability and indemnity Agreement and further agrees that no oral representations, statements, or inducements apart from the foregoing written Agreement have been made.

I acknowledge that I have read the foregoing paragraphs and I am completely aware of the potential dangers incident to engaging in the Activities, and fully aware of the legal consequences of agreeing to this Liability Waiver.

I HAVE READ THE ABOVE AND AGREE WITH ITS CONTENTS ENTIRELY AND AM SIGNING UNDER MY OWN FREE WILL AND CONSENT.

I have been offered by Club Alpha, copies of all documents I have signed (membership form, privacy agreement and liability waiver) and have for my own purposes and reasons have, without interference, chosen not to obtain or request copies of these documents.

I HAVE READ THE ABOVE AND AGREE WITH ITS CONTENTS ENTIRELY, AND ARE SIGNING UNDER MY OWN FREE WILL AND CONSENT.

Your Member ID will be used to identify you as a member of the Club Alpha.

Please remember your Member ID as it will be required each and every time you enter Club Alpha premise.

Failure to provide your member ID will result in having to complete these forms again.
